Repair Request form

Mr Bachmann;
I successfully got logged onto this Forum but I am utterly unable to get the Repair Request form . I am not getting responses from you if/when I try to register for the forum nor am I able to chanage to a password you will accept. Every response is an error message.
Can I just send a locomotive back with a letter without the ____ form?
